Vaccination Information:
If your child is entering Kindergarten or 6th grade OR is new to St. Monica, please send his/her vaccination records to the school nurses. Students entering 6th grade need to show evidence of receiving a TDAP and Meningococcal Vaccine. These are usually given at the 11 year old check up. Students entering Kindergarten need to show evidence of receiving their final DTaP, Polio, MMR and Varicella vaccines which are usually given at the 4 or 5 year old check up. If your child is still due to receive their vaccinations, please take a moment to schedule an appointment with their health care provider now. The Marion County Public Health Department also provides vaccinations but due to COVID-19 is only offering them by appointment. Vaccination records can be emailed to the school nurses at nurse@smsindy.org or paper copies can be sent into school to our attention.
